Obtain Documents Required for Matricula Accounts

Before you can open a North Shore Bank Matricula Checking or Savings Account, you will need to provide a Consular ID Card, and your Individual Tax Identification Number (ITIN). To obtain these documents, simply follow the instructions below.

  1. Consular ID Card

  2. To obtain this card, contact:
    Consulado General de Mexico

    In order to obtain a Consular ID card, you will need to provide any two of the following:

    • Original birth certificate.
    • A utility bill with your name and address..
    • Wisconsin Non-Drivers License ID.
    • U.S. Passport.
    • Photo County ID card.
    • An alien registration card.
    • College photo ID card.
    • Photo employee ID card.

  3. Plus You will need an Individual Tax Identification Number (ITIN)
  4. If you do not have an Individual Tax ID Number, please complete a W-7 form to request this number. (Please note that you will need to have Adobe Acrobat Reader installed on your computer to download and view the W-7 form. If you do not have this program installed, you may download it free of charge.)

  5. Take the completed W-7 form along with your Consular ID Card to the IRS office in the

    Federal Building - 2nd Floor 310 W Wisconsin Ave. Milwaukee, WI 53203.

    Plan on a time period of 4-6 weeks to receive the W-7 identification number.

To open your North Shore Bank Matricula Savings or Checking Account bring your Consular ID and your Individual Tax Identification Number (ITIN) to any branch office.
